Publisher 3DRealms and developer Anshar Studios have revealed the latest trailer for Painkiller, which reveals the new Rogue Angel game mode. Rogue Angel is a roguelike game mode featured in Painkiller, and it will be a separate mode from the main campaign of the title. In Rogue Angel, you will arrive in a brand-new realm of Purgatory in which you and your friends will advance through procedurally generated arenas, collect tarot cards, weapons, and items, and fight big bosses.

Painkiller is returning to life with this brand-new title. Set in Purgatory, you have been sentenced for your transgressions against Heaven, but the Voice of the Creator is giving you another chance to redeem yourself. You will blast through hordes of demons in brutal, fast-paced combat using a decent selection of infernal weapons. The game is set against the backdrop of gruesome gothic environments as you try to stop the fallen angel Azazel’s invasion of Earth. The game is playable in solo and co-op mode for up to three players.
If you are interested in playing Painkiller when it is launched next month, pre-orders for the game’s Standard and Deluxe Editions are now live across all platforms. Painkiller is also receiving a physical edition for PS5 and Xbox Series X|S at major retailers. Pre-orders will receive the ‘Iron Crusade’ pre-order bonus skin pack.
Physical pre-orders will also include the Burnished Pride weapons pack. The Deluxe Edition includes the Season Pass with the Night Watch DLC offering four characters and six weapon skins, and an assortment of weapon and character skins post-release. Painkiller launches on PS5, Xbox Series X|S, and PC via Steam on October 21, 2025.
