This Silent Hill: Townfall guide explains how to solve the organ transplant box puzzle and open it easily. The puzzle requires a few items from around the area, so follow the route below to get everything you need before entering the final code.
Organ Transplant Box Puzzle Guide – Silent Hill: Townfall
Begin near the phone booth by the town square and follow the street ahead. Continue along the road until you reach the small flag shown along the route, then turn right and take another right afterward; this will bring you to Zoe’s house.
Once you are inside, take the first right and search the cabinet in that room. You will find a flashlight there.
After picking it up, go upstairs and look for the picture. Enter the room on the left; another cabinet contains flashlight batteries.
Open your inventory and combine the batteries with the flashlight so it can be used. Once you have done that, head into the bathroom and check the floor mat where you will find the organ transplant card needed for the puzzle.
You cannot activate the generator yet because you are missing the required electricity card. Go back downstairs and check the hidden cabinet underneath the stairs where the generator is located. Since the power cannot be restored at this point, leave the house and retrace your route.
As you reach the end of the alley, a cutscene will play. Once it finishes, you will be back outside, but an enemy will now be nearby. You need to enter the building around the corner, although the way inside requires you to move around to its rear.
As you approach, the game will introduce the peek and hide mechanic. Wait for the enemy to move away from the doorway before going inside. If you are spotted, you can still make your way into the building. There is a wooden plank on a table inside that can be used as a weapon. You can block with Alt 2 and attack with R2.
After dealing with the enemy, check the cash register; the electricity card will be there along with a note. Take the card and return to Zoe’s house.
Solving the Organ Transplant Box Code
Go back to the generator room beneath the stairs and place the electricity card into the card machine.
Switch the machine to the card credit option, then press B to use the available credit. Once this is done, activate the master switch to restore electricity to the house.
Go towards the room on the left from the entrance, or to the room to the right when viewed from the electrical room. There you will find a computer. Interact with it and check the three messages available on the screen.
The second message contains the important information for the puzzle. It explains that the combination comes from the first three digits and the final three digits of the code printed on the transplant card.
Take out the card you collected earlier and examine it closely. Look at the right side of the card for the code labelled Y015. The first three digits are 956, while the final three digits are 133.
Return to the organ transplant box in the kitchen and enter the combination using those two sections of the code: 956 + 133. Make sure to confirm the unlock after entering the numbers.
The box will then open and trigger another cutscene, completing the organ transplant box puzzle.
